Purpose :
Purpose: To examine utility of broad-range real-time polymerase chain reaction (PCR) using preoperativelycollected aqueous humor as an appropriate diagnostic specimen for detecting pathogens of viral endophthalmitis
Methods :
Methods: This retrospective observational study enrolled 18 eyes of 18 patients (males / females; 12 / 6, average age ± SD; 60.4 ± 14.9 years old), who were clinicallydiagnosed with viral endophthalmitis. This study wasand The University of Tokyo Hospital in Japan between May 1, 2007 and December 1, 2017. Preoperative aqueous humor was collected at the time from 1 day to 7 days before therapeutic pars plana vitrectomy. After the collection of aqueous humor, valaciclovir treatment was performed. At the vitrectomy, undiluted aqueous humorand/or vitreous fluid were collected. Broad-range real-time PCR was used for detecting pathogens in the samples. If aviral load was more than 10 to the 4th power copy/ml in the samples, the virus was considered to be the pathogen of viral endophthalmitis. ruskal–Wallis H test was used for nonparametric comparison of multiple groups. Results :
Results: The enrolled patients were roughly classified into four groups as follows: 1) a group diagnosed by preoperatively or perioperatively collected aqueous humorand/or vitreous fluid (total group), 2) a group diagnosed by preoperatively collected aqueous humor (preoperative AH group), 3) a group diagnosed by perioperatively collected aqueous humor (perioperative AH group), 4) a group diagnosed by vitreousThere was no significant difference in age, gender and distribution of causative viral species among the four groups. The detection rates of total group, preoperative AH group, perioperative AH group and vitreous group were 94.1%, 91.7%, 100% and 60%, respectively. There were no significant differences of detection rates among the four groups (P = 0.085), and between preoperative AH groupand vitreous group (P = 0.135). fluid (vitreous group).
Conclusions :
Conclusions: These findings suggest that broad-range real-time PCR using preoperatively collected aqueous humor is an appropriate diagnosis method in viral endophthalmitis.
