Abstract
Purpose :
To investigate the exercise habits of those with primary open angle glaucoma (POAG) and its association with the progression of visual field (VF) loss.
Methods :
Daily physical activity (PA) was monitored by an accelerometer (ActiGraph wGT3x-BT), which patients wore for 24-hours on their waist (right) for one week during their regular visits in Wenzhou Glaucoma Progression Study. Moderate to vigorous physical activity (MVPA) time was the main outcome measure. All subjects had no less than three times reliable visual field test, and VF progression was defined as statistically significant deterioration on pattern deviation change probability maps in the same three or more points with confirmation by at least two consecutive visits detected by the Glaucoma Progression Analysis--likely progression or possibly progression.
Results :
The median (Interquartile range [IQR]) of better eye VF mean deviation (MD) was -1.75 (5.26) dB in the non-progressive group and -3.99 (9.08) dB in the progressive group (p=0.11). No significant difference was found for daily PA between the 71 non-progressive and 27 progressive patients (p>0.05 for all). 24-hour MVPA exercise habits showed that POAG patients had similar variation trends, 3 wave peaks and 2 wave hollows in the non-progressive and progressive group. The MVPA time was 19.89±15.81 minutes and 21.62±15.1 minutes during 07:00-09:00 am (p=0.20), 15.4±14.49 minutes and15.67±12.43 minutes during 15:00-17:00 pm (p=0.82) and 17.26±21.11 minutes and 11.42±11.58 minutes during 18:00-20:00 pm (p=0.001) in the non-progressive and progressive group respectively. Binary logistic regression analysis indicated that MVPA time (18:00-20:00 pm) (OR (95%CI) =0.82(0.73, 0.92)), MD (OR (95%CI) =0.94(0.92, 0.97)), mean arterial pressure (MAP) (OR (95%CI) =0.98(0.97, 0.99)), age (OR (95%CI) =1.06(1.03, 1.08)) and gender (OR (95%CI) =0.67(0.48, 0.96)) were significantly correlated with patients' progressive VF damage. Multivariate analysis showed that MVPA time per ten minutes (18:00-20:00 pm) (odds ratio, OR (95% confidence intervals, CI) =0.86 (0.76, 0.98)) were associated with progressive VF loss even after adjusting for other risk factors.
Conclusions :
The amount of evening exercise is associated with progressive VF damage, with ten minutes increase of MVPA time corresponding to a decrease in POAG progression by 14%.
