Usefulness of a simplified self-checking tool (Quattro CHART) for visual field defects

Abstract

Purpose : We developed a simplified self-checking tool installed in a tablet terminal (Quattro CHART) to help those possible patients to notice their visual field (VF) defects and evaluated its clinical usefulness.

Methods : The Quattro CHART is a multi-stimulus-type self-checking VF screening sheet using the flickering target of 15 Hz. Four test targets are displayed simultaneously in vertical and horizontal symmetry with each target displayed in each quadrant of the VF, which helps the examinee to self-check whether VF abnormality is present or not. This prospective study enrolled 35 eyes of 21 patients with glaucoma (11 males, 10 females, mean age, 57.9±12.1 years) and 20 eyes of 20 visually normal individuals (6 males, 14 females, mean age, 48.7±15.9 years), totaling 55 eyes of 41 subjects. The Quattro CHART iPad and Paper Editions, CLOCK CHART®, HFA SITA-Standard 30-2 were used to measure VF defects. The Quattro CHART Paper Edition carried the same arrangement of the test targets on the paper. We calculated the sensitivity and specificity of the Quattro CHART (iPad and Paper Editions), and CLOCK CHART®. The patients with glaucoma were classified into stages of glaucoma using MD and Greve’s Modified Method of the Aulhorn Classification (Aulhorn Classification) and the sensitivity according to each stage was obtained.

Results : The sensitivity of the Quattro CHART iPad Edition by MD was 91% when MD ≧ -6 dB, 100% when MD was -12 dB ≦MD ≦ -6dB, and 100% when MD ≦ -12 dB. The sensitivity of the Quattro CHART, iPad Edition by the Aulhorn Classification was 100% for Stage I and 100% for Stages II and higher. The sensitivity of the Quattro CHART Paper Edition was 74% for Stage I and 100% for Stages II and higher. In addition, the sensitivity of the CLOCK CHART® was 57% for Stage I and 100% for Stages II and higher. The specificity for the normal subjects was 100%.

Conclusions : The Quattro CHART is a method that helps possible patients to notice VF abnormality in a short time and is useful as a self-checking tool for VF abnormality.
