The Influences of Dry Eye Disease on Optical Quality

Abstract

Purpose : DED could result in visual disturbance and damage optical quality. We aimed to evaluate the influences of dry eye disease (DED) on optical quality and their correlations after a short period of treatment.

Methods : Twenty-five dry eye patients were recruited from July 2017 to July 2018 in Department of Ophthalmology, Peking University Third Hospital. DED and optical quality were analyzed before and after two or three weeks of treatment. Examinations for optical quality included National Eye Institute 25-Item Visual Function Questionnaire (NEI VFQ-25), contrast sensitivity, corneal high-order aberrations (HOAs), and some parameters of corneal power. Comparisons between data before treatment and data after treatment were performed by paired-samples t test. Correlations between parameters of DED and parameters of optical quality were analyzed with Spearman correlation coefficient.

Results : The Ocular Surface Disease Index (OSDI), tear film breakup time (TFBUT), corneal fluorescein staining (CFS) and best corrected visual acuity (BCVA) improved after a short period of treatment (all P<0.01). The total score of NEI VFQ-25 increased (P=0.016). Contrast sensitivity (CS) in photopic, photopic with glare, scotopic and scotopic with glare conditions were all increased (all P<0.05). Total anterior corneal HOAs over 6 mm analytical zones decreased (P=0.028). Objective scattering index (OSI), surface asymmetry index (SAI), average corneal power (ACP), standard deviation of corneal power (SDP), and irregular astigmatism index (IAI) decreased after treatment (all P<0.05). Changes of SAI, ACP, SDP and IAI respectively correlated well with changes of CFS (P=0.027, P=0.039, P=0.001, P=0.003, respectively). Changes of these parameters of cormeal power didn't correlate with changes of TFBUT (all P<0.05).

Conclusions : The optical quality of dry eye patients could improve after a short period of treatment. It’s possibly because that a decrease of CFS after treatment made a symmetrical corneal power. The sign CFS, but not TFBUT, may have a prominent influence on optical quality in DED.
