Abstract

Purpose : Spectral-domain optical coherence tomography (SD-OCT) allows the detection of exsudative and non exsudative signs in eyes with age-related macular degeneration (AMD). The purpose of this study was to describe and analyse a new SD-OCT sign that we called «subretinal evanescent hyporeflectivity » (SEHR).

Methods : Consecutive patients presenting between may 2017 and july 2017 with AMD (early and late neovascular) who had a SEHR without exsudative signs on SD-OCT were included. Clinical examination and SD-OCT (Spectralis, Heidelberg Engineering, Heidelberg, Germany) were analyzed at the inclusion and monthly, starting from one month before until 2 months after inclusion.

Results : Twenty-three eyes of 22 patients were included. Three eyes had early AMD and 20 late exsudative AMD: 16 eyes with type 1 macular neovascularization (MNV), 3 with type 2 MNV and 1with type 3 MNV. Among eyes with early AMD, none developed exsudative signs on SD-OCT during the 2 months of follow-up. Among eyes with neovascular AMD, 15/20 (75%) did not develop exsudative signs on SD-OCT whereas 5/20 (25%) developed exsudation: 4 eyes (3 type 1 MNV and 1 type 2 MNV) presented exsudative signs at 1 month and one eye at 2 months (type 2 MNV). SEHR was absent on SD-OCT one month before in these 5 eyes and disappeared on month after. Concerning the remaining 15 eyes with exsudative AMD, SEHR was visible one month before in 2 eyes, it disappeared 1 month after in 10 eyes, 2 months after in 2 eyes and remained visible at 2 months in 3 eyes. Among the 3 eyes with early AMD, SEHR was absent 1 month before and disappeared 1 month after.

Conclusions : SEHR is a new SD-OCT sign in patients with AMD. It should not be considered as an exsudative sign as it is not related to exsudation in 75% of eyes with neovascular AMD and could even be present in eyes with early AMD.
