Abstract
Purpose :
Scleral suturing of the Bausch & Lomb Akreos AO60 intraocular lens (IOL) is rapidly growing in popularity. There is very limited data on the refractive outcomes achieved using this technique. The purpose of this study was to evaluate refractive outcomes after scleral-sutured Bausch & Lomb Akreos AO60 IOL implantation.
Methods :
This was a single center, retrospective series of consecutive patients undergoing scleral-sutured Akreos IOL implantation. CPT codes were used to identify all patients who had undergone scleral fixated or scleral-sutured IOL implantation between 7/1/2011 and 2/28/18. Clinic charts, operative reports, and pre-operative IOL calculations were reviewed. Post-surgery data was collected at the 1-month follow up. In patients with longer follow up, additional data was collected at the 3, 6, and 12-month intervals. The pre-operative IOL targets and post-operative spherical equivalent refractive outcomes were collected and compared. The pre-operative IOL targets were identified based upon the IOL power calculation formula used and the power of the IOL implanted. Post-operative refractive outcomes were collected using the spherical equivalent of the last manifest refraction. The main study outcome was the difference between the pre-operative refractive target and the post-operative refractive outcome.
Results :
Twenty-seven patients who underwent scleral-sutured Akreos IOL fixation surgery during the study period were included. Of these 27 patients, 20 had a post-operative refraction in their medical records. Of those 20 patients, 7 (35%) had post-operative refractions that were within +/- 0.5 D of the pre-operative target refraction, 9 (45%) were within +/- 1.0 D of the target. Ten subjects (50%) were within -1.0 to -2.0 D of the target post-operative refraction. One subject (5%) had a hyperopic outcome within +1.0 to +2.0 D of the pre-operative target refraction.
Conclusions :
Among the 20 patients, 15 subjects (75%) had a more myopic refractive outcome than anticipated. Five subjects (25%) had a more hyperopic result than the target pre-operative refraction. Eleven subjects (55%) were greater than +1 or -1 diopter away from the target refractive error. Further studies evaluating the refractive outcomes of scleral-sutured Bausch & Lomb Akreos AO60 IOL will aid surgeons with their surgical planning and refractive targets to result in more predictable refractive outcomes.
