To evaluate the endothelial cells loss after phakic intraocular lens implantation at 1 year of follow up according to iris configuration assessed pre and postoperatively with the anterior segment OCT (AS-OCT).
Prospective, cohort study that included 60 eyes of 35 patients, who underwent phakic IOL implantation (Artisan, Ophthec) for surgical correction of myopia at Oftalmosalud Instituto de Ojos, Peru, between July 2016 to July 2018. Pre and postoperatively at 1 year of follow up, visual acuity, refraction and endothelial cell density (ECD) using specular microscopy was performed. Eyes were divided into 3 groups: plane, concave and convex according to iris configuration assessed with the AS-OCT. Safety distance was defined as the minimum distance between the edge of the optic and the endothelium, to prevent the risk for corneal dystrophy is 1.50 mm. Statistical analysis was performed using the R program, Wilconcox signed rank test and Kruskal Wallis test for means comparisons.
Mean endothelial cells loss were -204.79 ± 192.94 cell/mm2 , -307.50 ± 346.708 cell/mm2 and -197.18 ± 185.86 cell/mm2 in the plane, concave and convex group respectively. Mean safe nasal distance to the endothelium was -0.18 ± 0.30 mm, -0.3625 ± 0.378 mm and -0.1403 ± 0.375 mm in the plane, concave and convex group respectively. Mean safe temporal distance to the endothelium was -0.2421 ± 0.3297 mm, -0.3637 ± 0.3724 mm and -0.2885 ± 0.477 mm in the plane, concave and convex group respectively. There was greater endothelial loss in the convex group; however, there were no statistical significant differences.
The rate of endothelial cells loss after 1 year of implantation of phakic intraocular lenses is not associated with the configuration of the iris; it is recommend extend the study with a larger sample size.
