To detect early signs of staphylomas in children and young adults by ultra wide-field OCT.
A total of 42 highly myopic eyes (22 patients aged < 20 years; group I) and 137 highly myopic eyes (73 patients aged 20-49 years; group II) were examined by ultra wide-field OCT.
The mean axial length was 28.1mm in group I and 30.1mm in group II. Wide-field OCT detected staphyloma edges, characterized by an inward protrusion of the sclera with a gradual thinning of the choroid towards the staphyloma edge, in 4 of the 42 eyes (9.5%) in group I and in 60 of the 137 eyes (43.8%) in group II. Nine additional eyes (21.4%) in group I and 27 eyes (19.7%) in group II showed a gradual choroidal thinning from the periphery towards the predilection sites of staphyloma edges without scleral protrusions.
Gradual thinning of the choroid may be an early sign of staphyloma. Staphylomas, detected even in children, occur at a younger age than previously thought.
