Abstract

Purpose : To compare thickness of all macular intraretinal layers (mIRLs) between ocular hypertension (OHT) and early primary open-angle glaucoma (POAG) using spectral domain optical coherence tomography (SD-OCT) 8x8 posterior pole program (PPP).

Methods : 57 eyes of 57 OHT patients and 57 eyes of 57 early POAG patients were included. All of them had reliable PPP examinations (macular grid 8 × 8) drawn by SD-OCT (Spectralis, Heidelberg). Seven automatic segmentations were considered: macular retinal nerve fiber layer (mRNFL), ganglion cell layer (GCL), inner plexiform layer (IPL),inner nuclear layer (INL), outer plexiform + nuclear layer (OPLONL), photoreceptor layer (PRL) and retinal pigment epithelium (RPE). Thickness values were obtained on 64 cells of the grid for each mIRL, and mean thickness of superior and inferior hemispheres were calculated. Comparisons of mean thickness of these hemispheres and thickness of each cell between groups were determined. Significant differences in the cell by cell comparisons were represented quantitatively by heat maps for each mIRL.

Results : Thinning of superior and inferior hemisphere mean thickness on mRNFL, GCL and IPL and thickening on superior and inferior hemisphere mean thickness on PRL and inferior hemisphere on INL were found. Heat maps showed thinning patters in inner retinal layers (except for INL) and thickening patters in outer retinal layers in GPAA group.

Conclusions : PPP of SD-OCT allows to detect not only significant thinning patterns on inner retinal layers but also thickening patterns on outer retinal layers when comparing early POAG eyes to OHT eyes.
