Abstract
Purpose :
To investigate the frequency of newly arrived patients with uveitis (including scleritis) in central Tokyo area between January 2016 and July 2018 and compare them with those from 2004-2015.
Methods :
We retrospectively surveyed the records of patients with uveitis who first visited the Uveitis Clinic of the University of Tokyo Hospital located in central Tokyo between January 2016 and July 2018.Clinical data obtained included age, gender, diagnosis, anatomic location of inflammation (anterior, intermediate, posterior, pan), laboratory test results of blood and aqueous, chest X-ray and fluorescein fundus angiography findings.
Results :
From January 2016 to July 2018, 716 new patients (323 men, 393 women) with uveitis visited Tokyo University Hospital. Mean age was 56.2±19.0 years (men 55.9±19.7 years, women 56.7±18.0 years, respectively). Definite diagnoses were made in 441 cases (61.7%). The most common diagnoses were scleritis (7.6%), sarcoidosis (7.1%), herpetic iridocyclitis (5.7%, positive detection of herpes simplex virus, varicella zoster virus, and cytomegalovirus-DNA by polymerase chain reaction of aqueous humor was obtained in 0.6%, 2.0% and 3.2%, respectively) , intraocular malignant lymphoma (5.0%), Vogt-Koyanagi-Harada disease (4.5%), Behçet disease (4.2%), Fuchs heterochronic iridocyclitis(2.4%), bacterial endophthalmitis (2.4%), Posner-Schlossman syndrome (2.2%), acute anterior uveitis (2.1%), HLA-B27-associated uveitis (2.1%). The most frequent unclassified uveitis was sarcoidosis-suspected (23.4%). The frequency of anterior uveitis, intermediate uveitis, posterior uveitis, and panuveitis are respectively 36.0%, 2.7%, 6.8%, 54.5%. When compared with our former findings of the year 2004-2015, the present series showed increasing trend of intraocular malignant lymphoma (3.8% to 5.0%). On the other hand, they showed decreasing trend of fungal endophthalmitis (2.0% to 1.0%), Posner-Schlossman syndrome (3.1% to 2.2%). The frequency of anatomic localization of uveitis was not quite different among the studies conducted in 2004-2018.
Conclusions :
Increasing trend of intraocular malignant lymphoma might be connected with the increasing of aging population and patient referral.
