Comparison of astigmatism value with Barrett online calculator by using different measuring instruments in aged related cataract patients

Abstract

Purpose : To compare the difference of the corneal astigmatism taken with IOL master, Pentacam measurements, and Barrett on-line formula for astigmatism of age-related cataract.

Methods : Keratometry (K) and astigmatism (KA) were measured by IOLMaster, simulated keratometric refractive power (SimK) and astigmatism (SimKA) were measured by Pentacam, and the corresponding Barrett-KA (Barrett-KA) was obtained by Barrett on-line calculator. Meanwhile, total corneal refractive power (TCRP) and net corneal curvature (TNP) were measured by Pentacam. Patients in the study were divided into three groups, including with-the-rule(WTR) astigmatism group, the against-the-rule(ATR) astigmatism group and the oblique astigmatism group. The comparison between the measurements of astigmatism (IOLmaster KA, SimKA, TCRPA and TNPA) and the estimated values (Barrett-KA, Barrett-SimKA) was performed.

Results : After Barrett's formula calculation, KA and SimKA in WTR group and oblique astigmatism group decreased, increased in ATR group, and the axial position also changed. The centroid mean of the estimated total corneal astigmatism Barrett-KA(1.13±0.83×86°), Barrett-SimKA(1.02±0.91×91°)were less than the measured total corneal astigmatism TNPA (1.50±1.16×91°)and TCRPA(1.60±1.25×91°)in WTR eyes. The centroid mean of the estimated total corneal astigmatism Barrett-KA(1.84±0.83×1°), Barrett-SimKA(1.84±0.83×1°)were more than the measured total corneal astigmatism TNPA (1.00±1.12×5°)and TCRPA(1.10±1.18×5°)in ATR eyes. There was no significant difference between Barrett-KA (0.37±0.99×4°),Barrett-SimKA (0.32±0.86×12°)and TNPA (0.24±1.27×68°),TCRPA(0.1±1.38×108°) in oblique eyes. In WTR eyes, the centroid error of IOL Master KA and TCRPA was 0.25D, aligned along the vertical axis, while those between Barrett KA ,Barrett-SimKA and TCRPA were 0.54D and 0.61D respectively, aligned all along the horizontal axis . In ATR eyes, the centroid error of IOL Master KA and TCRPA was 0.77D, aligned along the vertical axis, and those between Barrett KA ,Barrett-SimKA and TCRPA were 0.40D and 0.3D respectively, aligned all along the horizontal axis .

Conclusions : There are some differences between the measured corneal astigmatism, the total corneal astigmatism and the estimated total corneal astigmatism measured by different devices. Further research is needed in clinical application.
