Abstract

Purpose : To assess the associations and diagnostic value of parapapillary beta zone and gamma zone with glaucoma in an elderly Chinse population.

Methods : Within the population-based Beijing Eye Study, glaucomatous eyes with optic disc radial scan were enrolled (n=177), 723 emmetropia and myopia individuals without retinal or optic nerve disease were selected as control group. Using optical coherence tomography (OCT), parapapillary beta zone and gamma zone were measured. Glaucomatous optic disc atrophy was assessed by the fundus photographs.

Results : The study included 114 patients with open angle glaucoma (OAG) , 62 patients with angle-closure glaucoma (ACG), and 723 control individuals. Both the prevalence of Beta zone and the mean width of largest beta zone was significantly higher/ larger in OAG and in ACG groups than in the control group (P<0.002). However, no difference was found in the prevalence or the width of Gamma zone between OAG / ACG and the control group (P>0.05). After adjusting for age and intraocular pressure, the prevalence (P=0.037) and width (P=0.016) of Beta zone were independent risk factors for the glaucoma discrimination, while the prevalence (P=0.72) or width (P=0.66) of gamma zone was not. The area under the ROC curve was 0.704 (95%CI: 0.660,0.748) for the glaucoma differentiation with the largest width of beta zone. The largest widths of Beta zone and of Gamma zone were significantly positively correlated (P<0.001). [MOU2] The location of the largest Beta zone located mostly in the temporal region in 459(87.4%) control eyes, and in the temporal region in 111(69.8%) glaucomatous eyes. The largest gamma zone located mostly in temporal region in 99(52.1%) control eyes, and in inferior region in 30(51.7%) glaucomatous eyes.

Conclusions : The presence and size of OCT-based Beta zone was an independent risk factor for glaucoma discrimination in the elderly population, however not for the Gamma zone.
