Abstract

Purpose : The anatomical relationship between Bruch’s Membrane Opening Minimum Rim Width (BMO-MRW) and circumpapillary Retinal Nerve Fiber Layer (cpRNFL) thicknesses determined by the spectral domain Optical Coherence Tomography (sdOCT) has not yet been thoroughly determined. Thus, the objective of this study is to investigate if the clinical delimitation of the optic nerve head (ONH) boundaries improves the relationship between BMO-MRW and cpRNFL.

Methods : This cross-sectional study evaluated 22 eyes of 11 healthy subjects (53.2 ± 19.6 years old; spherical equivalent: -0.3 ± 2.9 D) that underwent sdOCT examination (Spectralis, Heidelberg Engineering, Heidelberg, Germany). After acquiring the sdOCT scans for establishing both the BMO-MRW and the 3.5 mm cpRNFL, a manual edition was performed by an ophthalmologist (VJBF) based on his best clinical discretion for the delimitation the ONH rim boundaries, using en face images. Correlation coefficients between cpRNFL and BMO-MRW before ("automatic") and after ("clinical") edition were calculated for each Garway-Heath sector and compared with those computed with the Heidelberg database.

Results : Measurements taken from the Heidelberg database showed highly significant correlations for all Garway-Heath sectors, with the highest coefficient for the temporal-inferior sector (Table). Clinical delimitation of the ONH boundaries showed a slightly higher correlation between BMO-MRW and cpRNFL thicknesses for the temporal superior and inferior, nasal, and nasal superior as well as for the global values than the automatic delimitation (Table).

Conclusions : The anatomical correlation between BMO-MRW and cpRNFL thicknesses was overall improved after a clinical delimitation of the ONH, including the temporal superior and inferior sectors. Further studies, including both larger sample size and circumferential determinants of this relationship, may help improve the accuracy of glaucoma diagnosis.
