The Differences in Anterior Chamber Cellular Density, Size and Intensity of Uveitis Subtypes as Measured by Anterior Segment Optical Coherence Tomography in Uveitis Patients

Abstract

Purpose : To determine the anterior segment optical coherence tomography (AS-OCT) differences in cellular density, size and intensity in anterior uveitis patients with sarcoidosis, juvenile idiopathic arthritis, HLA B27 disease and infectious etiologies.

Methods : This is a retrospective review of patients with active and inactive anterior uveitis who had AS-OCT imaging of the anterior chamber. A customized algorithm was used to calculate the cellular density (number of cells/mm3) and measure the reflectance intensity and cellular size of each identified cell. Noninfectious cases of HLA-B27, sarcoidosis, juvenile idiopathic arthritis (JIA) related uveitis as well as various infectious cases of uveitis were imaged.

Results : 398 eyes overall were imaged from a set of 111 patients, with variability between the type of uveitis and the amount of anterior chamber inflammation. The average density measured 9.80 cells/mm3 for HLA B27, 0.99 cells/mm3 for sarcoidosis, 3.001 cells/mm3 for JIA and 5.17 cells/ mm3 for infectious uveitis. (ANOVA, p =8.8E-08). The differences amongst the groups was also significant (ANOVA, 1.2399E-07 < 0.05), when limiting to those with only 2+ cell or worse.
Cell intensity and cell size measurements were calculated for those with sarcoidosis and JIA anterior uveitis. The average cell intensity measured 904.51 for sarcoidosis and 1139.16 for JIA (ANOVA, p value=0.018), and the average cell area was 199.50 and 262.12 (ANOVA, p=0.013).

Conclusions : AS-OCT may be used to image and grade the degree of activity in anterior uveitis. Patients with HLA B27 tended to have the largest number of cellular density of the subtypes inflammation. When comparing JIA to sarcoidosis patients, JIA uveitis patients had larger cells and higher OCT reflectance. Differences in cellular measurements on AS-OCT could be used to differentiate between different etiologies of types of anterior segment inflammation.
