Abstract
Purpose :
It is known that foveal macular thickness and area of foveal avascular zone differs between sex. Thus we started this project to investigate whether there is sex differences of macular findings in idiopathic macular hole (MH) stages 3 and 4, in which the effect of the vitreous traction in the macula area is limited.
Methods :
Retrospective observational study. The subjects with MH stage 3 and 4 who underwent vitrectomy at Kagoshima University were enrolled. Age, preoperative visual acuity, degree of metamorphopsia using M-chart (Inami co, Tokyo, Japan), axial length and optical coherence tomography (OCT) parameters such as minimum macular hole diameter and bottom diameter in the horizontal and vertical directions were investigated from the medical records. Above factors between men and women were compared by the man-whitney U test.
Results :
Subjects were 34 eyes of 33 men (Stage 3 -27 eyes, stage 4 -7 eyes) and 48 eyes of 45 women (Stage 3 -30 eyes, stage 4 -18 eyes). The mean age was 66.6 ± 6.5 years old for men which was significantly older than women with the mean age of 63.5 ± 7.2 years old (p = 0.03). There were no significant differences in preoperative visual acuity (0.63 ± 0.21 for men and 0.65 ± 0.25 for women, p = 0.64), M-chart (0.81 ± 0.42 for men and 0.78 ± 0.49 for women, p = 0.76), axial length (23.8 ± 6.5 for men and 23.5 ± 0.88 for women, p = 0.08), or minimum macular hole diameter (Horizontal: 325.9 ± 96.2 for men; 307.3 ± 99.5 for women; p = 0.63 Vertical: 300.1 ± 102.3 for men; 281.3 ± 111.6 for women; p = 0.42). However, the bottom diameter of MH was significantly larger in men than in women (Horizontal: 808.4 ± 234.5 for men; 674.6 ± 209.6 for women; p = 0.034 Vertical: 715.3 ± 186.7 for men; 608.4 ± 214.5 for women; p = 0.045).
Conclusions :
In MH stage 3 and 4, the bottom diameter of the hole is larger in men than in women. There might be a difference of adhesive force in vitreo-retinal interface between sex in macular area.
