Abstract

Purpose : To compare the vertical and horizontal metamorphoosia in idiopathic macular hole (MH) stages 3 and 4, in which the effect of vitreous traction in the macula is limited, and to examine the relationship between the metamorphopsia and preoperative findings.

Methods : Retrospective observational study. We enrolled the subjects with MH stage 3 and 4 who underwent the surgery at Kagoshima University Hospital. Age, preoperative visual acuity, horizontal and vertical degree of metamorphopsia using M-CHARTS (Inami Co, Japan), axial length were investigated from medical records. In addition, minimum horizontal and vertical macular hole diameter and base diameter were determined from preoperative optical coherence tomography images. The relationships between the degree of metamorphopsia and above factors were assessed with pearson's rank correlation coefficient.

Results : Subjects were 34 eyes of 33 men (Stage 3 -27 eyes, stage 4 -7 eyes) and 48 eyes of 45 women (Stage 3 -30 eyes, stage 4 -18 eyes). The mean age was 63.2 ± 12.1 years and preoperative visual acuity was 0.65 ± 0.32. The degree of metamorphopsia was significantly larger in the vertical direction than in the horizontal direction (0.86 ± 0.48 in vertical, 0.72± 0.47 in horizontal, p = 0.018). The minimum macular hole diameter and the bottom diameter were both larger in the horizontal direction (macular hole diameter: horizontal 315.037 ± 98.0, vertical 288.9 ± 107.6, p = 0.058; bottom diameter: horizontal 727.5 ± 232.0, vertical 651.7 ± 209.2, p = 0.03). In terms of the relationship between the degree of metamorphopsia and the above factors, statistically significant correlations were found between the vertical metamorphopsia and age (r=-0.23, p=0.03) and between the horizontal metamorphopsia and vertical macular hole diameter (r=0.23, p=0.04).

Conclusions : In MH stage 3 and 4, the metamorphopsia is larger in the vertical direction than in horizontal direction. Horizontally wider shape of the hole might contribute the symptoms and pathology in MH patients.
