Abstract

Purpose : Sub-foveal choroidal thickness (SFCT) evolution over time and its risk factors in the elderly are poorly known. The objective of this study is to assess normative data of SFCT evolution in French elderly subjects and to determine ocular and cardiovascular factors influencing its rate of change.

Methods : The Alienor study is a population-based cohort of residents of Bordeaux (France), followed every two years since 2006. The present study is based on 282 participants aged 77 years of more, with at least 2 eye examinations from 2011 to 2017. SFCT was measured manually from EDI-OCT (enhanced depth Imaging-Optical coherence tomography) examinations. Evolution of SFCT over time and its associations with age, sex, ocular parameters and vascular risk factors were assessed using multivariate mixed linear models, with random effect to take into account intraindividual correlations between eyes.

Results : Mean age of the participants was 82.3 years (standard deviation (SD): 3.3). Mean SFCT at baseline was 208.5 µm (SD: 77.7) ranging from 17 to 457 µm. Overall, SFCT decreased by 10.8 µm per year (p<0.0001). At baseline, SFCT was significantly thinner in older subjects (-4.6 µm/year of age, 95% confidence interval (CI) [-7.12; -2.20]), in women (-19.09 µm 95% CI [-35.78; -2.41]), participants with axial length > 26 mm (-21.8 µm CI [-27.98; -15.58]), with Intra-Ocular Pressure (IOP) greater than the median (>13 mmHg) (-14.73 µm/mmHg; CI [-26.5; -2.95]), and in left eye (-7.09 µm; CI [-13.77; -0.40]). Evolution over time was slower for women (+2.41 µm/year 95%CI [0.50; 4.33]), and for participants with diabetes (+3.26 µm/year; CI [0.07; 6.46]), but was not significantly associated with other factors. In the final multivariate model, only diabetes was significantly associated with a slower decrease of SFCT with time (+3.27 µm/year; 95%CI [0.07; 6.47]).

Conclusions : In this cohort of elderly participants, mean SFCT decreased over time. At baseline, SFCT was significantly thinner in older participants, in women, in participants with high axial length and those with high IOP, and in left eyes. The rate of evolution of choroidal thickness was slower in women and in participants with diabetes but not with other factors. Other studies are needed to confirm these results.
