Abstract

Purpose : To investigate the association between mean deviation of visual field (VF) measured by Humphrey field analyzer (HFA) and corneal shape, thickness, and elevation parameters measured by CASIA.

Methods : We included 6,086 eyes of 3,142 patients with 462 corneal parameters including shape (topography), thickness (pachymetry), and elevation of anterior and posterior chambers measured by CASIA instrument (Tomey, Japan). A total of 466 eyes had mean deviation (MD) of VF measured less than 18 months apart from the corneal parameter measurements. We computed the correlation between MD and each of 462 corneal parameters independently and measured their statistical significance. We then investigated the significance of those corneal parameters that showed strong correlation with MD (after Bonferroni correction) using generalized linear mixed-effects model (GLMM; Fig.1).

Results : Out of 462 corneal parameters, 12 corneal shape and elevation parameters were independently correlated with MD (P value < 0.00002). Using GLMM, only five parameters were strongly associated to MD. Keratoconus Prediction Index (KPI; a discriminant function calculated from numerous corneal parameters) had a strong negative correlation with MD (R =0.91). The root mean square (RMS) of the anterior elevation at 5 and 3 mm, Klyce/maeda keratoconus index (KCI), as well as anterior elevation score had a moderate correlation with MD (R= 0.57, 0.53, 0.50, and 0.43, respectively). Age was negatively correlated with MD.

Conclusions : This study demonstrated that global VF loss is significantly correlated with KPI, KCI, RMS and anterior elevation score parameters of corneal topography and elevation. No significant correlation was found between MD and pachymetry parameters. The corneal parameters that are significantly correlated with global VF loss may be useful in identifying glaucomatous VF severity and patients with glaucoma.
