Abstract

Purpose : Routine eye exams are recommended for those 65 and older, and anyone with diabetes, a family history of glaucoma or current eye disease. This study aims to assess the frequency of appropriate eye care utilization among patients who participated in a free eye disease screening program at two low-income community clinics in Michigan.

Methods : This study includes 1,171 adults who participated in the Michigan Screening and Intervention for Glaucoma and Eye Health through Telemedicine (MI-SIGHT) program during the first year of recruitment. Participants were surveyed on their health history (previous diagnoses of diabetes or glaucoma, family history of glaucoma) and history of eye care use including the last time their eyes were examined by an eye care provider and the last time they had a dilated eye exam. Participants received a comprehensive eye disease screening via telemedicine to identify glaucoma/suspected glaucoma, cataract, diabetic retinopathy (DR), and age-related macular degeneration (AMD). Data were summarized using descriptive statistics (frequency, percentage).

Results : MI-SIGHT participants were on average 55 years old, 38% were male, 54% Black, 10% Latinx, 33% had a high school education or less, and 70% reported <$30,000 household income. Of those participants aged 65 or older, 151 (55.5%) reported not having an eye exam in two years or more. In those with self-reported diabetes, glaucoma, or a family history of glaucoma, 214 (51.0%), 14 (31.8%), and 130 (53.3%) reported not having an eye exam in two years or more, respectively. Of those participants who screened positive for glaucoma, cataract, DR, or AMD, 137 (51.0%), 129 (53.6%), 38 (45.8%), and 9 (40.9%) reported not having an eye exam in two years or more, respectively. Further, for those with glaucoma or DR, 174 (76.3%) and 51 (72.9%) reported having their last dilated eye exam over 1 year ago, respectively.

Conclusions : The utilization of recommended, routine eye care is concerningly low among this cohort of participants which includes mostly individuals with low incomes.
