Abstract

Purpose : Retinopathy of prematurity (ROP) is the leading cause of childhood visual impairment worldwide. Many believe that ROP only affects patients during its active phase. Emerging evidence shows that ROP carries protracted impact on one’s vision. To date, comprehensive data on long-term outcomes on ROP remain scarce. Here, we performed a prospective, observational, cohort study to evaluate the long-term impact of ROP on visual function and ocular structure in school-aged children.

Methods : Recruited pediatric patients were divided in three groups: former premature babies diagnosed with ROP (“Ex-ROP”) (average age=12.4 year-old [yo]), former premature babies without ROP (“Preterm”) (average age=11.9 yo) and full-term age-matched controls (“Full term”) (average age=9.7 yo). Participants underwent a complete ophthalmological examination including visual acuity, intraocular pressure (IOP), refraction, color vision and biometry. Retinal anatomy was assessed via optical coherence tomography (OCT) and retinal function was measured by multifocal and flash electroretinogram (ERG). ANOVA followed by two-tailed Student’s or Welch’s t-test were used (p<0.05).

Results : Astigmatism was significantly increased in Ex-ROP subjects compared to Preterm (p=0.006) and Full-term subjects (p=0.012). IOP was significantly higher in the Ex-ROP group (20.38 mmHg) compared to the Preterm group (16 mmHg, p=0.011). There was no significant difference in BCVA, color vision or axial length. The Ex-ROP group showed a significantly thicker foveal and parafoveal regions compared to the other 2 groups. Attenuation in retinal function was observed with several ERG parameters in the Ex-ROP group.

Conclusions : Our study indicated that a history of ROP is associated with an increased IOP, macular thickening and diminished retinal function in a group of older children. These data support the notion that ROP is not a static disease, but a dynamic condition capable of long-standing impacts. Therefore, close follow-ups are warranted in patients with a history of ROP.
