Abstract

Purpose : Johnson and Johnson Surgical Vision (JJSV) is developing a new femtosecond (nFS) laser. This study describes the development of the nFS laser flap, and its quality assessment in terms of ease of flap lift compared to iFS, the current commercial flap for LASIK (Laser In Situ Keratomileusis) maker

Methods : A prospective, multicenter, open-label clinical study was performed at 3 sites. One eye of 174 subjects was treated with nFS and fellow eye with commercially available iFS. Improvement in the ease of flap lift was evaluated by surgeons’ grading of nFS vs iFS following each surgery as follows: 1) iFS is much better than nFS, 2) iFS is superior to nFS, 3) iFS is similar to nFS, 4) nFS is superior to iFS, 5) nFS is much better than iFS
Several iterations were performed in the development of a LASIK flap creation with nFS to achieve a flap similar or better than iFS. Iterations included changes in energy levels and/or flap creation patterns such as addition of a pocket and better channeling of the opaque gas bubbles (OBL) towards the pocket

Results : At early phases of the study, venting of OBL was not optimized leading to minor stickiness during flap lifts. With improvements in flap creation pattern, the addition of a pocket, energy optimization, and better channeling of OBL, the nFS ease of flap lift grading by surgeons increased from 33% of cases being equal or better than iFS to 100% of the cases. Further, after optimization, 72% of nFS flap gradings of ease of flap lift were better than iFS

Conclusions : Laser flap creation patterns and energy levels for the next generation femtosecond laser were optimized through clinical iterative process, achieving equal or superior ease of flap lift compared to iFS
