Abstract
Purpose :
Historically, macula-off retinal detachments (Mac-off RD) were seen as less urgent as compared to macula-on retinal detachments (Mac-on RD) or vitrectomy due to endophthalmitis. At this ASC, Sunday is not part of the typical work schedule and surgeons should operate on only the most compelling cases. With annual review of cases per surgeon instituted 5 years ago, the impact of the reviews and trend in case types are reported. The frequency of these three urgent interventions in an ophthalmic ambulatory surgery center (ASC) during weekend call hours was analyzed using a retrospective chart review of the past 5 years.
Methods :
The ASC practice management software was used to collect all patient records who underwent retinal surgeries on Sundays from 2018 to 2022. The number of Mac-on RD, Mac-off RD, and vitrectomy for endophthalmitis surgeries for the year were recorded. Percentages of each type of surgery per year were calculated and analyzed for trending.
Results :
In 2018, 59.6% (28) of emergency retinal surgeries were Mac-on cases, 27.7% (13) were Mac-off, and 12.8% (6) were endophthalmitis. Over the past 5 years Mac-on cases increased by 6.15% per year, Mac-off cases decreased by 4.46% per year, and endophthalmitis cases decreased by 1.69% per year. In 2022, 89.7% (35) emergency retinal surgeries were Mac-on cases, 5.1% (2) were Mac-off, and 5.1% (2) were endophthalmitis.
Conclusions :
Over the past 5 years, macula-on retinal detachment surgeries have trended to being a greater portion of urgent cases and macula-off have trended to a lesser portion of urgent cases. This appears to correspond with the instituting of annual reviews per surgeon on case urgency and selection for weekend surgery.
