Abstract

Purpose : This study aimed to compare retinal blood flow and vascular resistance between diabetic macular edema (DME) and normal eyes using laser speckle flowgraphy (LSFG).

Methods : Between April 2019 and January 2022, 50 normal eyes (36 men’s eyes, mean age; 60.3 years) and 40 DME eyes (22 men’s eyes, mean age; 63.3 years) were examined at Kagawa University Hospital. Patients with retinal diseases except diabetic retinopathy, glaucoma, and treated with intravitreal injections with vascular endothelial growth factor inhibitors or sub-Tenon’s capsule steroid injections were excluded. The mean blur rate (MBR) of large vessels in the optic papilla and total capillary resistance (TCR) were measured during the first visit using LSFG. The TCR receiver operating characteristic (ROC) curve was drawn and the DME diagnostic ability of TCR was determined using the area under the curve (AUC). The TCR cutoff value was determined using the Youden index.

Results : The MBR between normal (33.4±6.9) and DME eyes (31.0±10.5) was not significantly different (p = 0.19). The TCR was significantly higher in DME eyes (0.95±0.3) than that in normal eyes (0.71±0.26) (p < 0.01). The TCR AUROC curve was 0.74, indicating that it is moderately accurate in diagnosing DME. Using the Youden index, the TCR cutoff value was 0.79 (sensitivity 0.74, specificity 0.70).

Conclusions : TCR values may be used as diagnostic aids for diabetic macular edema.
