Abstract

Purpose : A recent international panel created a new consensus based OCT definition of LMH, MPH, and ERMF using specific OCT criteria (Hubschman 2020). The surgical outcomes using this consensus definition have been described in only one small retrospective case series (Chehaibou 2021). The purpose of this study is to characterize the change in visual acuity for eyes treated with PPV based on the disease classification using the new consensus based OCT definition.

Methods : Retrospective chart review was performed on patients from 2000-2022 of a single surgeon at a community hospital. Patients who underwent PPV for LMH, MPH, or EMRF in at least one eye and have spectral domain OCT imaging were included. Preoperative OCT imaging were reviewed for each patient to determine the classification of their retinal disease based on the current consensus guidelines’ major and minor criteria. The primary outcomes were mean change in best corrected visual acuity (VA) at 3 months, 1 year, and final exam postoperatively.

Results : A total of 51 patients met the inclusion criteria, 30 with LMH, 14 with MPH and 7 with EMRF. The mean preoperative VA for all patients was 20/62, and at post-operative month 3, year 1, and at time of final follow up was 20/56, 20/40 and 20/44 respectively. Comparing preoperative VA and final postoperative VA, there was no significant improvement in patients with LMH (p=0.38), but there was significant improvement in patients with MPH (p=0.04), and EMRF (p=0.02). The results for all patients and each subgroup are summarized in table 1 and figure 1.

Conclusions : There is no significant improvement in VA at final post-operative visit after PPV in patients with LMH, while there is a significant improvement in patients with MPH and EMRF. However there are some limitations to this retrospective study, including limited sample size and the surgeon’s choice about which eyes were most likely to benefit from surgery. The results support surgery in selected eyes with MPH and EMRF, but possibly not in eyes with LMH.
