Abstract
Purpose :
To investigate changes of intraocular pressure and anterior segment parameters after operation and compare differences among cataract surgery, vitrectomy and combined operation.
Methods :
Medical records of patients who underwent cataract surgery, vitrectomy or combined operation were reviewed retrospectively. Preoperative and postoperative BCVA, IOP, refractive changes were investigated. Anterior segment parameters including central corneal thickness (CCT), anterior chamber depth (ACD), lens thickness (LT), angle-to-angle (ATA) distance, and lens vault (LV) and anterior chamber angle measurements at 500 and 750 μm including angle opening distance (AOD), trabecular-iris space area (TISA), and trabecular-iris angle (TIA) were measured using anterior segment OCT. The axial length was checked before surgery using partial coherence interferometry.
For statistical analysis, Chi square test, ANOVA, independent T-test with Bonfferroni correction, paired T-test, Pearson correlation analysis were performed. The significance level was p < 0.05.
Results :
There were 41, 15, and 40 eyes in the cataract (cat), vitrectomy (vit), and combined (combi) surgery groups. In the baseline characteristics, there was no difference in each group, but the cat group was older than the combi group (p> 0.004). There was a significant decrease in IOP at 6 months in the cat and combi groups. (p = 0.004, p = 0.002). CCT increased at 1 and 6 month after surgery in the cat and combination groups (p<0.005, respectively). ACD increased at 6 months in all groups (p < 0.030, respectively). The LV moved backward at 6 months in the cat and combi groups (p < 0.001, both). AOD, TISA, and TIA were increased at 6 months in the cat and combi groups(p < 0.02, respectively). Changes in IOP were correlated with preoperative AOD, TISA 750, and TIA in the cat and combi groups (p <0.03, respectively). In the cat and combi groups, preoperative ACD and LV were correlated with changes in AOD, TISA 750, and TIA 75 (p < 0.05, both; rho <-0.43, rho > 0.33).
Conclusions :
The combined and cataract group showed a decrease in IOP and an increase in AOD, TISA, and TIA after surgery. In these two groups, preoperative ACD and LV status were related to the anterior chamber angle measurements changes. Postoperative IOP changes are thought to be due to changes in the condition of the anterior chamber due to crystalline lens removal.
