Corneal Aberrometic changes after ICL V4C intraocular lens implantation

Abstract

Purpose : To measure the change in higher-order aberration (HOA) of Implantable Collamer Lenses (ICL) V4C

Methods : We included eyes that underwent ICL V4c and Toric ICL V4c implantation for myopia and myopic astigmatism between March 2023 and September 2023. Patients over 21 years old who fulfilled surgical criteria were included. Corneal aberrations were measured by Pentacam AXL at 6mm and the total corneal aberrations were taken. The normality test used was Shapiro-Wilk. Depending on normality, the data were processed with T-Student for Paired data or Wilcoxon.

Results : 25 eyes from 13 patients were included. 68% were female patients. The mean corneal aberrations before and after ICL V4C implants at a diameter of 6mm are shown in table 1. The variables with normal distribution were Z1-1, Z22, Z2-2, Z33, Z3-1, Z44, Z42, Z4-2, Z4-4, Z53, Z51, Z5-3. The aberrations with a statistically significant (p <0.05) difference between preoperative and postoperative measurements were Z2-2, Z44, Z51, Z20 and Z40.

Conclusions : HOAs changes after ICL V4c insertion were: Defocus, Oblique Astigmatism, Vertical Quadrafoil, Primary Spherical and Horizontal Secondary Coma. Despite the fact that many of the distributions did not follow a normal pattern, the statistically significant shift observed in Z40, an aberration that has recently been associated with improved optical performance and increased depth of focus, is particularly noteworthy. We still do not know if this change has been due to the making of the corneal incisions or due to the change in the ocular optical system when implanting an intraocular lens.
