Abstract
Purpose :
With the prevalence of DR increasing rapidly, we infer that diabetic vitrectomy also had an increasing trend in recent years, but there is still a lack of real-world research evidence with large sample data in multiple provinces in China. We use the Hospital Quality Monitoring System (HQMS) to assess the number of end-stage vitrectomy surgeries.
Methods :
This was a retrospective study based on the nationwide HQMS database, which collects information from the front pages of in-hospital medical records. DR patients with vitrectomy were enrolled. All diagnoses and operations were identified by the International Classification of Diseases version 10 (ICD-10) diagnostic codes and surgical operation classification codes . Descriptive analyses were used to determine the numbers of vitrectomy and changing trends over 6 years.
Results :
145317 hospitalisation records of 130622 diabetic vitrectomy patients, with 51.45% male, were identified for analysis, of what 124375 (95.22%) patients were from tertiary hospitals, whereas 6247 (4.78%) were from second-class hospitals. There were 64286 patients who underwent diabetic vitrectomy and intravitreal injection during a single hospitalization. 31638 patients who underwent vitrectomy more than once or bilaterally during 6 years, with 27239 underwent twice and 3808 underwent 3 times. The annual amount of diabetic vitrectomy has increased from 9999 in 2016 to 34272 in 2021, and has maintained an upward trend for 6 years. The mean age of patients having diabetic vitrectomy increased from 55.42±10.26 years in 2016 to 56.26±10.48 years in 2021. The 50-59 age subgroup accounted for 38.44% of all age. Diabetic vitrectomy amount in 6 years varied by geographic regions, with the lowest amount in North-West (8.66%), followed by South-West (8.77%), North-East (9.65%), Central (12.30%), North (13.95%), South (15.94%), and East (30.74%). The top amount provinces are Guangdong, Shandong and Henan, which is corresponding to the resident population rank. The ambulatory surgery rate is 11.32%. The length of stay decreased from 9.71±8.23 days in 2016 to 7.37±5.84 days in 2021, and has maintained an downward trend for 6 years. Moreover, the total hospital expense, out-of-pocket expense and its propotion are $2528.91±1316.84, $1070.62±1313.29, 0.4254±0.3718.
Conclusions :
Our study indicates that diabetic vitrectomy had an increasing trend from 2016 to 2021.
