Abstract
Purpose :
To examine the cataract detection ability of a simple visual field checker (MEOCHECK) based on retinal projection technology, which is unaffected by adverse conditions in the cornea and does not require mydriasis.
Methods :
Refraction tests, visual acuity tests, intraocular pressure tests, and Humphrey Field Analyzer (HFA) 24-2 tests were performed and MEOCHECK was used on 73 eyes of 37 taxi drivers. Nuclear cataract (NUC) grading was performed according to the WHO Simplified Cataract Grading System. The association between the measured parameters and the MEOCHECK score was evaluated. MEOCHECK is a visual field measurement device based on the HFA 24-2 program that has a viewing angle of 42 degrees horizontally and 30 degrees vertically, with indicators presented every 6 degrees. It can measure one eye in a minimum of 56 seconds. At each measurement point, a score of 1 is assigned if the patient cannot respond to an indicator presented up to 3 times, and a score of 0 is assigned if the patient responds. Patients with intraocular lenses or eye diseases other than cataracts were excluded.
Results :
The mean values (± SD) for age, spherical surface, logMAR visual acuity, intraocular pressure, mean deviation (MD), pattern standard deviation, and foveal threshold in the 73 eyes were 60.5 ± 11.4 years, -1.1 ± 2.4 D, 0.09 ± 0.08, 13.1 ± 2.5 mmHg, -0.9 ± 1.3 dB, 1.9 ± 0.7 dB, and 35.8 ± 2.0 dB. A total of 17 eyes had NUC0 cataracts, 44 had NUC1 cataracts, 12 had NUC2 cataracts, and 0 had NUC3 cataracts. The combined NUC0 + NUC1 group had a significantly lower MEOCHECK score than the NUC2 group: 10.4 ± 7.7 vs. 23.2 ± 5.4 (p < 0.001, Wilcoxon single rank test). A receiver operating characteristic analysis of the NUC2 group revealed that MD had an area under the curve of 0.57 (p = 0.47), logMAR of 0.68 (p = 0.02), foveal threshold of 0.72 (p = 0.01), and MEOCHECK score of 0.89 (p < 0.001, cut off = 17). In addition, a logistic regression analysis showed that the odds ratio for cataracts with grade NUC2 for QDL score was 1.19 (95% CI 1.0-1.4, p = 0.02), and for age it was 1.28 (95% CI 1.0-1.6, p = 0.01) .
Conclusions :
MEOCHECK, a simple visual field measurement device, was more useful for detecting early cataracts than logMAR, MD, or foveal threshold.
