Abstract
Purpose :
The purpose of this retrospective study is to evaluate anterior and posterior
irregular astigmatism of limbal stem cell deficiency (LSCD) by Fourier harmonic analysis of
topographic data obtained by anterior segment optical coherence tomography
Methods :
The patients of LSCD who visited the University of Tokyo Hospital from April 2022 to October 2023 with the measurement of anterior segment optical coherence tomogprahy CASIA2, were included to this study. Control eyes were selected with no history of corneal or conjunctival disease, corneal astigmatism within -1.5D, CCT of 500-560μm, no history of HCL wear within one month, and BCVA>1.0 were included.
From the topographic data of anterior segment OCT, the spherical component, regular astigmatism, asymmetry, and higher-order irregularity were obtained using Fourier analysis for the anterior and posterior corneal surface and evaluated by stage.
Results :
The case group consisted of 25 patients with 25 eyes (16 males and 9 females, age 63 ± 15.8 years). The control group consisted of 47 patients with 47 eyes (26 males and 21 females, age 66.8 ± 9.5 years).
The disease stages of the cases were Stage I in 5 cases, Stage II in 9 cases, and Stage III in 11 cases. The causative diseases were SJS/TEN in 6 cases, chemical trauma in 6 cases, aniridia/chromosomal abnormality in 4 cases, facial nerve paralysis/eyelid closure insufficiency in 2 cases, ocular cicatricial pemphigoid in a case and unknown causes in 6 cases.
In the anterior surface, a significant increase in regular astigmatism, asymmetrical, and higher-order irregular astigmatism components was observed in Stages I, II, and III compared to the control. In the posterior surface, a significant rise in the regular astigmatism was noted solely in Stage II. Additionally, both the asymmetric component and the higher-order irregular astigmatism component show increases across Stages I, II, and III compared to the control. In LSCD cases, there was no significant correlation between anterior and posterior Fourier components and LogMAR best corrected visual acuity.
Conclusions :
In this study using Fourier analysis of anterior segment OCT, an increase in irregular astigmatism was observed in both the anterior and posterior corneal surfaces in LSCD, regardless of the stage.
