Abstract

Purpose : Primary Pars plana vitrectomy for RRD has a success rate of 90%. However even after a successful repair and a good recovery of visual acuity, patient can have a deficit of visual functions. The aim of this study was to assess visual functions (subjective and objective) in patients who had undergone surgery for rhegmatogenous retinal detachments followed by silicone oil removal (SOR) and had a subsequent visual acuity of 20/33 (0.22 logMAR) or better

Methods : This was a prospective, non-interventional pilot study. 8 patients who had undergone a pars plana vitrectomy with silicone oil tamponade for a RRD followed by SOR and had a final visual acuity of 20/33 or better were evaluated for visual functions including reverse polarity, contrast sensitivity, colour sensitivity, flicker sensitivity and aniseikonia. The photopic ERG provided the objective functional measurement. OCT and fundus images were used to assess structural integrity. The fellow eye was also evaluated and was used as a control. The findings in the eye with RRD and the healthy control eye were subsequently compared.

Results : A decreased contrast sensitivity in the affected eye across all spatial frequencies (~3-60 cycles/°) with mean AULCSF (Area under the log contrast sensitivity curve ) value of 1.15 ± 0.19 was noted. Color sensitivity in the affected eye was reduced, with mean CAD units of 3.10 ± 1.96 for R-G defect and 3.80 ± 2.50 for Y-G defect. The flicker sensitivity was decreased across all the five locations with mean central FMT (flicker modulation thresholds) percentage of 9.24 ± 5.37 in the affected eye. Aniseikonia test revealed macropsia in four patients (3 macula-off RD and 1 macula-on RD) and micropsia in two patients (macula-on RD). Significant reduction in the mean photopic 3.0 and photopic flicker ERG amplitudes was found in the affected eyes compared to the healthy eyes (p<0.05).

Conclusions : Our study reveals abnormalities in spatial, temporal, and colour domains in both subjective and objective assessments in eyes with rhegmatogenous retinal detachment with good post operative visual acuity when compared to the healthy eyes.
